In the first document, Prison Warden J. Warren Mead and other witnesses certify that "Leon F. Czolgosz alias Fred Nieman" was "executed by causing to pass through his body a current of electricity of sufficient intensity to cause death." It is signed by Mead and Sheriff Samuel Caldwell, and on a separate sheet by 12 other witnesses, most of them members of the jury. Also included is an autopsy report signed by Carlos MacDonald and John Gerin, who certify that "we attended the execution of the said Leon F. Czolgosz alias Fred Nieman and immediately thereafter, performed an autopsy upon the body of the same and find that the cause of death was due to a current of electricity passed through his body."
