[40], 696, [3] pages, including final leaf with colophon dated 1621; lacks V5 and 2K4 (both with illustrations). Text printed in red and black throughout. Interleaved and sporadically annotated in ink with prayers in English. 4to, early 19th-century calf gilt with brass catches and clasps, flat spine divided by false bands into 6 compartments, lettered in one with Holy Monogram in the rest, joints crudely reinforced with leather; light dampstaining in upper margin of opening leaves, repaired clean tear across C3 not affecting text on recto or illustration on verso, ink annotations in margins of 2 full-page illustrations.
