Single sheet approximately 8x10.5 inches, being pages 3-47 and 3-48 from the flight plan flown on the mission with notes made during the flight by Neil Armstrong and Michael Collins. Page 3-47 has 14 item checks and 6 data entries made by Armstrong. Those data entries are: "75 41 23" which is the LOS (Loss Of Signal) time in mission elapsed time (hh mm ss) as sent to Armstrong by Mission Control. signed and inscribed by buzz aldrin. This document represents the exact time the CSM and LM docked configuration would lose contact with Earth as it passed behind the Moon. Entries were made by Armstrong for AOS (Acquisition Of Signal) times after the first LOI (Lunar Orbit Insertion) burn at "76 15 29" if completely successful and "76 05 30" if not successful. If the burn did not occur, the crew would have looped around the Moon ten minutes earlier than planned and headed back to earth on a "free return trajectory".
The updated time for the first LOI burn of "75 49 4965" was entered by Armstrong with the flight plan printed time marked out as well as the correct BT (Burn Time) time of "6 +02" and delta VT (change in velocity) of "2917.3". But the most significant mark by Armstrong is a line marking out the NO GO part of the GO/NO GO line for the first LOI burn. This was a command decision only Armstrong could make while his crew was out of contract with Mission Control.
On page 3-48, Collins who actually fired the engine for the LOI burn, has entered "Roll 180 (degrees), Acq MSFN (V64), then pitch down to 315 local horizontal". This was the maneuver that Collins made to have the CSM high gain antenna in proper alignment to communicate with the Manned Space Flight Network (MSFN). Armstrong made data entries in the Burn Status Report block including the actual BT (burn time) of "5" (min) and "57" (sec), Fuel (usage) of "38.8" and Ox(idizer) usage of "39.0". He notes the orbit parameters of "169.9" by "60.9" nautical miles as well as SPS nitrogen A and B tank levels. Mission Control had concerns with the B level during communications on the first lunar orbit. Armstrong has made over 18 data entries and two item checks on this page.
