Field Grade Officer Effectiveness Report. A single page, two sided form for evaluating the job performance of then USAF Major Charles Duke while assigned to NASA. The period covers part of 1967 and early 1968. The reporting officer was Astronaut Leroy G. Cooper, Jr., and signed by him with concurrence signed by Astronaut D. K. Slayton.
