The program, for the service that took place at the Friends Meetinghouse in New York on November 30, 1996. 48 unnumbered pages including photograph bound in, with small color photograph tipped to inside rear cover and "Riverboat Casino" chip in envelope tipped to page 35 as issued. 4to, stiff wrappers, bound with ribbon; moderate dampstaining affecting rear cover, color photograph, and last 10 leaves. First limited edition, one of 500 bound copies (100 unbound).
