Octavo format manuscript on laid paper watermarked with a large crest and cursive initial W, dated 1794, comprising 197 numbered pages, followed by a number of blank leaves and three pages of index, taken by one of Rush's medical students at the University of Pennsylvania, disbound, in original boards (both detached, sewing perished), ex libris Dr. George Lee, with his book ticket and the bookplate of the Library of the University of Maryland's School of Medicine, first leaf of text with rubber library stamp, 7 x 4 1/4 in.
