Partially-printed document signed, 4 pages, 31 x 21.3 cm, on one folded sheet, with printing on first page only in 35 lines of Roman type, the remainder completed in manuscript; disbound with marginal loss, leaves nearly detached, worming near bottom affecting 2 words of printed text, skillfully conserved.
